Camp Photo Order
Use this form to order any of our Formal &/or Crazy Pictures and have them either sent to you via the US Postal Service or by email.
Team Camp Application Form w/Deposit (Coach/Mgr. Only)
This application is for TEAM CAMPS conducted by the Lords of Soccer for you club team or school. Please complete the registration form and submit it with your minimum non-refundable deposit of $200 per team. There is a $7.00 transaction fee for the online service. Thank you.
Team Camp Registration Form (Players Only)
This application if for TEAM CAMPS conducted by the Lords of Soccer for you club team or school. Please complete the registration form and submit it with your payment. Thank you.
Summer Camp DEPOSIT w/Paypal
This form allows you to apply the minimum deposit required for any camp, according to your camp preference, to insure your child's attendance. Please note that there is a $3 transaction fee that will be included. NOTE: IF YOU ARE PURCHASING EQUIPMENT, YOU WILL NEED TO GO TO THE CAMP STORE TO APPLY IT TO THE TOTAL PURCHASE PRICE.
Summer camp registration (Return by US Mail/ No fees)
Choose this application if you are RETURNING YOUR REGISTRATION BY MAIL ONLY. Please click on the link to begin the application process for your child to attend our summer camps and feel free to contact us if you have any questions. YOU WILL NEED TO RETURN A COPY OF THE APPLICATION TO US WITH YOUR PAYMENT. Thank you.
Summer camp registration (PAY ONLINE w/PAYPAL)
You may pay directly online through our Paypal system to help expedite your application process. Please note that there is a $5 regular camp transaction fee, applicable to your original payment.
